{"id":5167,"date":"2019-12-11T20:30:37","date_gmt":"2019-12-11T20:30:37","guid":{"rendered":"https:\/\/www.kusd.edu\/eschool-staging\/?page_id=5167"},"modified":"2026-02-18T12:23:40","modified_gmt":"2026-02-18T18:23:40","slug":"special-eschool-medical-concerns-program","status":"publish","type":"page","link":"https:\/\/www.kusd.edu\/eschool\/counselor\/special-eschool-medical-concerns-program\/","title":{"rendered":"Special eSchool Program Medical Concerns Program"},"content":{"rendered":"<h2 style=\"text-align: center\"><strong>February 18, 2026: The Kenosha eSchool 2025-26 MEDVI Program is full for grades 9-12.<\/strong><\/h2>\n<hr \/>\n<p><strong>Special eSchool Program for those with Medical Concerns<\/strong><\/p>\n<p>The Medically Necessary Virtual Instruction Program (MEDVI) provides instruction for KUSD students who are unable to attend school for at least one marking period (or more than 14 consecutive school days) in a face-to-face school environment due to a medically documented physical injury, medical condition, or severe emotional problem. The program is designed to provide support for core academic courses on a short-term basis. MEDVI is only offered in middle and secondary grades, and enrollment is always subject to the availability of courses. Students who are applying for this program must complete the Release of Information, Physician Verification, and Communication Agreement Forms and submit to Kim Gorman, the eSchool Program Counselor. She can be reached at <a href=\"mailto:kgorman@kusd.edu\">kgorman@kusd.edu<\/a> and 262-359-7764.<\/p>\n<h3>To apply for the MEDVI program, complete all 3 steps listed below:<\/h3>\n<h2>Step 1<\/h2>\n<p><strong> Complete the following four forms and submit them electronically.<\/strong><br \/>\nThe forms below can be filled out online from any device. A printable version is provided at the link below.<\/p>\n<ul>\n<li><a href=\"https:\/\/na4.documents.adobe.com\/public\/esignWidget?wid=CBFCIBAA3AAABLblqZhCn5zSkqMo4esmfl2mCRXbKBorD-b-jf4Tx8sSA8PZ64DqRSzrmlJfFUtDyVKfYV1o*\" target=\"_blank\" rel=\"noopener\">MEDVI Communication Agreement Form<\/a><\/li>\n<li><a href=\"https:\/\/na4.documents.adobe.com\/public\/esignWidget?wid=CBFCIBAA3AAABLblqZhAOdLh78UPAaAroYhTcD5BstKS5GSRTQqRqetROPLXeIDv8jmy82ZNVILFEkJVPRsk*\" target=\"_Blank\" rel=\"noopener\">MEDVI Program Information and Agreement Form<\/a><\/li>\n<li><a href=\"https:\/\/na4.documents.adobe.com\/public\/esignWidget?wid=CBFCIBAA3AAABLblqZhALoQ8O0BObaJ7eEu8UdPoo0Pcob979Y8M3C4Fu_XcwnjJo3AK9TsWtOK16FV63TEM*\" target=\"_blank\" rel=\"noopener\">MEDVI Release of Information Form<\/a><br \/>\nThis is the consent form that gives permission for appropriate eSchool staff to discuss the students&#8217; progress with the referring Physician or Therapist for issues related to the physical, emotional, or mental health concerns reported on the Physician\/Therapist Verification Form.<\/li>\n<li><a href=\"https:\/\/na4.documents.adobe.com\/public\/esignWidget?wid=CBFCIBAA3AAABLblqZhCAU9Kd1x2Kj21loq6isRlLIaa0lsxOODQLywIxjFM74iD8DTYRojIG_nv-uSdNrdM*\" target=\"_blank\" rel=\"noopener\">MEDVI Physician\/Therapist Verification Form<\/a><br \/>\n<strong>There are two steps for completing the Physician \/Therapist Verification Form.<\/strong><\/p>\n<ol>\n<li style=\"list-style-type: none\">\n<ol>\n<li>The parent completes the demographic information and signs the form electronically and submits it to eSchool as the other three forms were.<\/li>\n<li>\u00a0A copy of the Physician\/Therapist form submitted by the parent is emailed to the email address the parent provides. The parent should open the document, download the form, and print a copy to provide to the Physician or Therapist for he\/she to complete and sign off on with an original signature. <strong>It is the parents\u2019 responsibility to provide the form for the doctor or therapist for them to complete.<\/strong> The Physician\/Therapist must include the reason the student cannot attend their boundary school, a plan for medical or therapeutic services, and an estimate of the length of time services will be required. The completed form can be faxed to 262-359-3824, scanned and emailed to Kim Gorman at <a href=\"mailto:kgorman@kusd.edu\">kgorman@kusd.edu<\/a>, or mailed to Kenosha eSchool Program at 8518 22 Avenue Kenosha, WI 53143.<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.kusd.edu\/eschool\/wp-content\/uploads\/sites\/39\/2025\/08\/MEDVI-Print-Pack-082225.pdf\" target=\"_blank\" rel=\"noopener\"><strong>PRINTABLE MEDVI FORM PACKET<\/strong><\/a><\/p>\n<h2>Step 2<\/h2>\n<p><strong>Create a parent\/guardian account if the student has not taken an eSchool Program course in the past.<\/strong><br \/>\n<em>* If your child is new to the Kenosha eSchool Program, a parent\/guardian account must be created in order for the student to be enrolled in our required orientation course. All new students must have an account created and complete orientation in order to be enrolled in eSchool courses. Please create the account at <a href=\"https:\/\/wen.geniussis.com\/PublicWelcome.aspx?id=30\" target=\"_blank\" rel=\"noopener\">https:\/\/wen.geniussis.com\/PublicWelcome.aspx?id=30<\/a>. Once the account has been created and the MEDVI forms have been submitted, your child will be notified via e-mail that they have been enrolled in the orientation course.\u00a0 If you have questions regarding orientation, you may contact Steve Moreland at <a href=\"mailto:smorelan@kusd.edu\">smorelan@kusd.edu<\/a>.<\/em><\/p>\n<h2>Step 3<\/h2>\n<p>Once we receive all the required MEDVI forms and the parent\/student account is created, the student will be enrolled in our required orientation course. Communication regarding orientation and instructions will be sent via email. All students must successfully complete orientation within one week.<\/p>\n<h2>Next Steps<\/h2>\n<p>The eSchool Program counselor will review the MEDVI forms and the students&#8217; academic records. The principal of Kenosha eSchool Program makes placement determination. All placement decisions by the Kenosha eSchool Program principal are final. <strong>Students are still enrolled at their boundary school until placement decision has been made. Attendance will continue to be taken by the respective boundary school during the application process.<\/strong> If placement is granted, the parent will be notified and the following will occur.<\/p>\n<ul>\n<li>The student will be multiple school enrolled in the Kenosha eSchool Program. (primary placement listed in Infinite Campus remains at the student&#8217;s KUSD boundary or full time school of record as required by DPI.)<\/li>\n<li>Attendance will include a medical leave designation (MED) at Kenosha eSchool Program during the enrollment.<\/li>\n<li>After one semester in the program, a review of student progress will be made. A new\/updated medical verification form may be requested from the doctor or therapist at that time if necessary. <strong style=\"background-color: yellow\">Students who are not having success in their eSchool courses or do not comply with the medical or therapeutic services outlines in the plan, can be dropped from the MEDVI program and will need to return to their boundary area school.<\/strong><\/li>\n<\/ul>\n<h2>End of School Year<\/h2>\n<p>Students&#8217; MEDVI status expires at the end of the school year. Students are automatically assigned to return to their boundary school for the following school year. If the student wants to apply to eSchool as a regular student (not under the MEDVI program) for the next school year they should complete and submit the application at the beginning of April when the lottery for the next school year takes place. The student can also apply t a different choice or charter school in KUSD. If the student needs to apply for the MEDVI program again, <strong>all<\/strong> forms need to be updated and submitted for the new school year.
