Special eSchool Program for those with Medical Concerns<\/strong><\/p>\n The Medically Necessary Virtual Instruction Program (MEDVI) provides instruction for KUSD students who are unable to attend school for at least one marking period (or more than 14 consecutive school days) in a face-to-face school environment due to a medically documented physical injury, medical condition, or severe emotional problem. The program is designed to provide support for core academic courses on a short-term basis. MEDVI is only offered in middle and secondary grades, and enrollment is always subject to the availability of courses. Students who are applying for this program must complete the Release of Information, Physician Verification, and Communication Agreement Forms and submit to Kim Gorman, the eSchool Program Counselor. She can be reached at kgorman@kusd.edu<\/a> and 262-359-7764.<\/p>\n Complete the following four forms and submit them electronically.<\/strong> PRINTABLE MEDVI FORM PACKET<\/strong><\/a><\/p>\n Create a parent\/guardian account if the student has not taken an eSchool Program course in the past.<\/strong> Once we receive all the required MEDVI forms and the parent\/student account is created, the student will be enrolled in our required orientation course. Communication regarding orientation and instructions will be sent via email. All students must successfully complete orientation within one week.<\/p>\n The eSchool Program counselor will review the MEDVI forms and the students’ academic records. The principal of Kenosha eSchool Program makes placement determination. All placement decisions by the Kenosha eSchool Program principal are final. Students are still enrolled at their boundary school until placement decision has been made. Attendance will continue to be taken by the respective boundary school during the application process.<\/strong> If placement is granted, the parent will be notified and the following will occur.<\/p>\n Students’ MEDVI status expires at the end of the school year. Students are automatically assigned to return to their boundary school for the following school year. If the student wants to apply to eSchool as a regular student (not under the MEDVI program) for the next school year they should complete and submit the application at the beginning of April when the lottery for the next school year takes place. The student can also apply t a different choice or charter school in KUSD. If the student needs to apply for the MEDVI program again, all<\/strong> forms need to be updated and submitted for the new school year. \nThis is the consent form that gives permission for appropriate eSchool staff to discuss the students’ progress with the referring Physician or Therapist for issues related to the physical, emotional, or mental health concerns reported on the Physician\/Therapist Verification Form.<\/li>\n

\n* If your child is new to the Kenosha eSchool Program, a parent\/guardian account must be created in order for the student to be enrolled in our required orientation course. All new students must have an account created and complete orientation in order to be enrolled in eSchool courses.
